In South Africa, one of PRASA's brand new AFRO 4000 locomotives derailed together with a part of its train in the early morning of August 19. Fortunately, there were no casualties, but the locomotive has suffered heavy damage and a large number of people was injured. Images of the derailed locomotive can be found here: link.

Update: According to this image report (link) on the Friends of the Rail Forum, the loco in question was AFRO4010.
